18e301e101 IB/mlx4: Fix the size of a buffer in add_port_entries()

d68627697d treewide: Remove uninitialized_var() usage
commit 3f649ab728cda8038259d8f14492fe400fbab911 upstream.

Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

git grep '\buninitialized_var\b' | cut -d: -f1 | sort -u | \
	xargs perl -pi -e \
		's/\buninitialized_var\(([^\)]+)\)/\1/g;
		 s:\s*/\* (GCC be quiet|to make compiler happy) \*/$::g;'

drivers/video/fbdev/riva/riva_hw.c was manually tweaked to avoid
pathological white-space.

No outstanding warnings were found building allmodconfig with GCC 9.3.0
for x86_64, i386, arm64, arm, powerpc, powerpc64le, s390x, mips, sparc64,
alpha, and m68k.

ccf5a1b28e IB/isert: Fix incorrect release of isert connection
[ Upstream commit 699826f4e30ab76a62c238c86fbef7e826639c8d ]

The ib_isert module is releasing the isert connection both in
isert_wait_conn() handler as well as isert_free_conn() handler.
In isert_wait_conn() handler, it is expected to wait for iSCSI
session logout operation to complete. It should free the isert
connection only in isert_free_conn() handler.

When a bunch of iSER target is cleared, this issue can lead to
use-after-free memory issue as isert conn is twice released

13e29c379b IB/isert: Fix dead lock in ib_isert
[ Upstream commit 691b0480933f0ce88a81ed1d1a0aff340ff6293a ]

- When a iSER session is released, ib_isert module is taking a mutex
  lock and releasing all pending connections. As part of this, ib_isert
  is destroying rdma cm_id. To destroy cm_id, rdma_cm module is sending
  CM events to CMA handler of ib_isert. This handler is taking same
  mutex lock. Hence it leads to deadlock between ib_isert & rdma_cm
  modules.

- For fix, created local list of pending connections and release the
  connection outside of mutex lock.

87059a4df2 IB/hfi1: Fix SDMA mmu_rb_node not being evicted in LRU order
[ Upstream commit 9fe8fec5e43d5a80f43cbf61aaada1b047a1eb61 ]

hfi1_mmu_rb_remove_unless_exact() did not move mmu_rb_node objects in
mmu_rb_handler->lru_list after getting a cache hit on an mmu_rb_node.

As a result, hfi1_mmu_rb_evict() was not guaranteed to evict truly
least-recently used nodes.

This could be a performance issue for an application when that
application:
- Uses some long-lived buffers frequently.
- Uses a large number of buffers once.
- Hits the mmu_rb_handler cache size or pinned-page limits, forcing
  mmu_rb_handler cache entries to be evicted.

In this case, the one-time use buffers cause the long-lived buffer
entries to eventually filter to the end of the LRU list where
hfi1_mmu_rb_evict() will consider evicting a frequently-used long-lived
entry instead of evicting one of the one-time use entries.

Fix this by inserting new mmu_rb_node at the tail of
mmu_rb_handler->lru_list and move mmu_rb_ndoe to the tail of
mmu_rb_handler->lru_list when the mmu_rb_node is a hit in
hfi1_mmu_rb_remove_unless_exact(). Change hfi1_mmu_rb_evict() to evict
from the head of mmu_rb_handler->lru_list instead of the tail.

8fa3ee8ef1 IB/IPoIB: Fix queue count inconsistency for PKEY child interfaces
[ Upstream commit dbc94a0fb81771a38733c0e8f2ea8c4fa6934dc1 ]

There are 2 ways to create IPoIB PKEY child interfaces:
1) Writing a PKEY to /sys/class/net/<ib parent interface>/create_child.
2) Using netlink with iproute.

While with sysfs the child interface has the same number of tx and
rx queues as the parent, with netlink there will always be 1 tx
and 1 rx queue for the child interface. That's because the
get_num_tx/rx_queues() netlink ops are missing and the default value
of 1 is taken for the number of queues (in rtnl_create_link()).

This change adds the get_num_tx/rx_queues() ops which allows for
interfaces with multiple queues to be created over netlink. This
constant only represents the max number of tx and rx queues on that
net device.

[ Upstream commit cbe71c61992c38f72c2b625b2ef25916b9f0d060 ]

kmalloc_array() is called to allocate memory for tx->descp. If it fails,
the function __sdma_txclean() is called:
  __sdma_txclean(dd, tx);

However, in the function __sdma_txclean(), tx-descp is dereferenced if
tx->num_desc is not zero:
  sdma_unmap_desc(dd, &tx->descp[0]);

To fix this possible null-pointer dereference, assign the return value of
kmalloc_array() to a local variable descp, and then assign it to tx->descp
if it is not NULL. Otherwise, go to enomem.

48c78097a1 RDMA/rxe: Don't overwrite errno from ib_umem_get()
[ Upstream commit 20ec0a6d6016aa28b9b3299be18baef1a0f91cd2 ]

rxe_mr_init_user() always returns the fixed -EINVAL when ib_umem_get()
fails so it's hard for user to know which actual error happens in
ib_umem_get(). For example, ib_umem_get() will return -EOPNOTSUPP when
trying to pin pages on a DAX file.

Return actual error as mlx4/mlx5 does.

b8fc8bbfcc RDMA/cxgb4: Fix missing error code in create_qp()
[ Upstream commit aeb27bb76ad8197eb47890b1ff470d5faf8ec9a5 ]

The error code is missing in this code scenario so 0 will be returned. Add
the error code '-EINVAL' to the return value 'ret'.

Eliminates the follow smatch warning:

drivers/infiniband/hw/cxgb4/qp.c:298 create_qp() warn: missing error code 'ret'.

8d034c20b4 RDMA/rxe: Fix failure during driver load
[ Upstream commit 32a25f2ea690dfaace19f7a3a916f5d7e1ddafe8 ]

To avoid the following failure when trying to load the rdma_rxe module
while IPv6 is disabled, add a check for EAFNOSUPPORT and ignore the
failure, also delete the needless debug print from rxe_setup_udp_tunnel().

$ modprobe rdma_rxe
modprobe: ERROR: could not insert 'rdma_rxe': Operation not permitted
